Privacy Policy

1. Overview

The MRI Staff Screening App is operated by Children’s Nebraska to capture and review MRI safety screenings for staff and personnel who enter the MRI environment. This policy explains what information the app collects, how it is used, and how protected health information is safeguarded.

2. Information We Collect

We collect the staff name, medical record number (MRN), and badge identity captured by badge scan or manual lookup at the kiosk and mobile surfaces; responses to the 18-question medical-history safety screening; and the account email and role of the Radiology reviewers who access screening records.

3. Protected Health Information (PHI)

Screening responses are Protected Health Information. They are handled in accordance with HIPAA and Children’s Nebraska policy, are accessible only to authorized Radiology staff on the approved hospital network, and are never sold or used for advertising.

4. How We Use Information

Information is used to determine MRI safety eligibility, to let Radiology reviewers review, flag, and annotate screenings, and to meet clinical, safety, and legal obligations. We do not sell personal data or use it for any purpose unrelated to MRI safety screening.

5. Access Logging

Every time a reviewer opens or edits a screening record, that access is recorded in a PHI Access Log with the reviewer’s identity and the time of access, so that access to protected health information can be audited.

6. Data Retention

A completed screening form is valid for one year. A new submission supersedes the prior one; superseded history is retained for audit and safety review rather than deleted, and remains subject to the same PHI protections.

7. Your Rights

Staff may request access to, or correction of, their own screening information by contacting the Radiology Department. Where applicable law grants additional rights, those rights apply and requests are handled under the hospital’s HIPAA procedures.
